The uploaded documents must include your name and the name of the institution. Include a copy of the transcript legend or university key (back page in most cases) for each institution. Please view all uploaded documents to be sure the scanned copies are clearly legible and can be printed on letter size paper - 8.5 by 11 (21.5 by 30 cm). Do not mail a copy to the department.
