PS Personal Statement/Letter of Intent These instructions apply to the on-line UF Graduate Admissions application only! To complete your University Graduate On-line application, you must enter a personal statement. Simply copy and paste the statement that you wrote for your OTCAS application. The department will access your personal statement through OTCAS. GPA Minimum cumulative GPA of 3.0 (based on a 4.0 scale) for upper division (junior and senior level) undergraduate college coursework. Minimum GPA of 3.2 (based on a 4.0 scale) for all prerequisite courses. No grade lower than a C. Fee Apply online and submit UF's non-refundable application fee of $30.00. Credit card payments require an additional $1.75 processing fee. If unable to pay via credit card, please include the fee cover memo provided within the application and include your full name and UFID on the check or money order. RL Applicants must submit three (3) recommendations. One (1) of the recommendations must be from a professional level OT. The other two (2) should be from professionals, such as a present or former faculty member, academic advisor, or employer. Names and addresses do not need to be entered on University Graduate On-line application, but must be provided to the appropriate section of the OTCAS application. Transcript Transcripts The Office of Graduate Admissions must receive official transcripts from each post-secondary institution you attended. Former UF students do not need to request UF transcripts for Graduate School Admission.
